The Add Record Form is simply a web form that will post the information back to the Microsoft Access Database that resides on the web server.

The photo upload module is using a module for Macromedia Cold Fusion. It allows up to five images to be uploaded and once and allows a management screen to view and delete records. The file name is then referenced in a block in the add record block, and will display with the record.
